Closing the Loop

Program Description

The Older Adults Community Care Collaborative Initiative, or "Closing the Loop", is a pilot project which has been conceptualized off the Bridge Model of transitional care. This approach utilizes a social work based approach to providing community care. It has been successful in reducing hospital readmissions within 30 days of discharge, and the model serves as a framework from which this initiative was built upon.
