Tools and resources for older adults, families and caregivers
CATCH-ON, a team of health systems, community organizations and other partners, focuses on making sure that older adults get the best possible care in the health system and at home. We educate health care clinicians and caregivers, and we work with community-based organizations to help close gaps in care. We also work to promote Age-Friendly Health Communities (AFHCs) and Dementia-Friendly Communities (DFCs).
